In regards to rooms or room set up. Some rooms have chairs/tables already in them. Please indicate here if you will need to change that set up (be aware that you will need to return the room to its original layout once your event ends).
Note: if you desire to use any of the equipment in our Sanctuary, CCCOG will provide the individual(s) to run the equipment at a fee (see Facility Use Policy)
Regarding requested equipment
1.) I have read and accepted the CCCOG Policies and Procedures (found on cccog.com/facility) and agree to abide by the guidelines as stated;
2.) I will adhere to the deposits and fees as outlined in the Policies and Procedures and as described below; and
3.) Any keys checked out to me will be returned within 24 hours at the conclusion of my event.
